The Evaluation Question/Item Information dialog provides a place to enter/update information about questions.

| Code | Short-hand unique identifier for the Code that appears on data entry screens and in drop-down lists. |
| Abbreviation | Brief Description used during Evaluation Response Entry/Update and on Response Reports. |
| Full Question | Complete text of an Evaluation Question used during Evaluation Response Entry/Update and on Response Reports. |
| Description |
Narrative description of the item. |
| Age Category | Assigns a developmental age to the question. Used in in Feedback letters to report only age-appropriate items. |

Feedback |
Used to include/exclude a question from Feedback letters. This makes it possible to exclude negative behaviors from progress reports (e.g. "hits other children when angry"). |

Response Set |
Assigns a Response Set to the question. This field is not completed for Tools where all questions share the same response set.. |
