Evaluation Recording - Battery Information

A Battery refers to one or more evaluations that will be/were provided as a coordinated whole. A Battery can also be thought of as work order depending upon the specific situation. It documents who is being evaluated, when the work is being done, and an overall result.

The Result field can be used to record an overall outcome determined by a staff person after reviewing the actual responses, computed outcomes, and factors outside the computer system.

Description
Battery Type Specifies who was evaluated.

Project

Identifies the project being evaluated if applicable.

Provider

Identifies the provider being evaluated if applicable.

Client

Identifies the client being evaluated if applicable.

Staff Id

Identifies the staff being evaluated if applicable.
Time Point Determines if the evaluation was done at admission, while open, or at discharge. Each agency can further define time points to be more specific. Time Point codes are shared by the Service Plan section making it possible to associate evaluations at different phases of service to service plans.
Time Point Label User-defined description which accompanies the coded Time Point.
Status User-defined status code showing if the evaluation is complete or not.
Instrument User-defined/non-coded.
Result User-defined coded result for the entire battery. Can be used to record human eligiblity determination based upon Outcome Scores and other factors.
Begin Date Beginning of time period during which the evaluation(s) ocurred.
End Date End of time period during which the evaluation(s) ocurred.
Next Eval Date Date of next evaluation if applicable.
Note Narrative comments about the battery of evaluations as a whole.
